New Dental Patient Information – Orem, UT
Welcome to Your New Dental Home
Taking care of your smile doesn’t have to be complicated. Our team at Frandsen Dental of Orem is here to make every aspect of your visit as simple and straightforward as they can. We’ll be happy to answer any questions you might have about our office as well as what you can expect to happen during your first appointment with us. But before you call us, we invite you to review the information below to make sure that you’re properly prepared to visit our dental office in Orem.
Patient Forms
To make sure your paperwork is taken care of before your appointment, we recommend downloading the necessary new patient forms with the link below so that you can fill them out in your own time. Don’t hesitate to call us if the download isn’t working or if you’re not sure how to complete the forms.
Dental Insurance
If you have dental insurance, you can save money on your routine preventive appointments as well as any emergency services that might be required. We gladly take a number of plans, and we’re in-network with major providers like Delta Dental, BlueCross BlueShield, Dentist Direct, MetLife, and more. Even if you’re out-of-network, our team can still work with you to help you use your benefits to the fullest. If you contact our office, we can confirm whether we are in-network with your plan and answer any insurance-related questions that might be on your mind.
Join Our In-House Membership Plan
As part of our efforts to make our services as accessible as possible, we offer an in-house membership plan for patients without dental insurance. This plan covers any basic preventive care you might need, but it doesn’t come with any of the pitfalls associated with traditional dental insurance, such as deductibles or waiting periods. Feel free to get in touch with our office if you’re interested in learning more about our in-house membership plan; we’ll make sure that you have a clear idea of exactly what’s covered.
CareCredit Financing
Does your insurance not offer enough coverage for a specific procedure? Or do you not have insurance at all? In either case, you may want to think about applying for CareCredit financing. You won’t have to worry about fitting the full cost of your care into your budget; CareCredit makes things easier by letting you choose a low monthly payment plan that comes with little interest (or even no interest in some cases). Reach out to us today for more information about CareCredit and how to apply.
